Intermediate Care Facility for the Mentally Retarded (ICF/MR)

 

 

 

 

A responsible direct care staff is on duty and awake on a 24-hour basis when people are present. Each person receives a continuous active treatment program which includes training, health services and related services that help him/her function with as much self determination and independence as possible.

Family and Community Living (FCL)
   
Family and Community Living is a residential service which provides support to people living in a home owned or leased by the person. Services may include training and assistance in personal care, budgeting, shopping, laundry, etc. Levels and amounts of support may vary depending on the person’s needs. The person is responsible to pay for room and board.
ISLA (Individualized Supported Living Arrangement) means people living, working, playing, and being a part of the community just like other people without disabilities. The main focus of ISLA is to help people achieve the highest level of independence s/he is capable of achieving and to be
as actively involved in the community as s/he desires. Because every person has individual needs and desires, each person we provide services to defines the type of service s/he needs or wants and how many hours s/he needs. Services provided include teaching people how to care for their medical needs, how to manage their finances, safety skills, caring for their home, shopping, cooking, planning recreational activities, etc. In addition to choosing the type and frequency of services, people choose where and with whom they live.

People have the option of living in apartments, condominiums, trailer homes, or purchasing and owning their own homes. They also choose whom they want to live with or choose to live alone. We assist people in taking advantage of the many opportunities there are in our community for recreational activities. People attend sporting events, plays, craft shows, rodeos, concerts, fairs, etc.

People are assisted in taking their dream vacations. Examples include country concerts in Branson, Missouri; a dude ranch in Arizona; visiting a sister in New Mexico; visit a mother in Florida;

Dollywood in Nashville, Tennessee; Disney World in Miami, Florida; camping and fishing in Oregon; shopping at the Mega Mall in Minneapolis; a Disney Cruise to the Bahamas; etc. People also are inclusive members of their communities and demonstrate this through their participation in sporting clubs, horseback riding groups, health clubs, church groups, Creative Memories members, etc. In addition to participating in the community, people are encouraged to give back to the community through volunteer work, i.e., delivering flowers at hospitals, Adopt-A-Pet, participating in the annual MS walk and cancer walks, stocking shelves at the Food Pantry, helping with the Giving Tree, etc.

 

Minimally Supervised Living Arrangements (MSLA)

Individuals live in a townhome that they share with a friend. Staff is on-duty on a 24 hour basis. As with all our other residential programs, each person receives a continuous active treatment program, health services and related services which assist him/her to develop skills necessary to live an independent life style as much as possible.

People with disabilities that are living in their own town home through the MSLA (Minimally Supervised Living Arrangement) allows them to live in smaller environments with more choices and flexibility in their daily life. People with disabilities receive staff support and are responsible for rent, groceries, utilities, etc. This environment enables people to live in a small environment in the community with staff support through their entire day so they are able to participate in the life of their community.

The MSLA program began operations on June 1, 2003. As with any new program and as we prepare to expand, we are accepting donations of all household items (i.e., furniture, small kitchen appliances, kitchen gadgets, bathroom accessories, etc).
